• Do I need a prescription for physical therapy?
A prescription is required for treatment. In Michigan, a physical
therapist can perform an evaluation without a prescription but needs
a referral from your physician in order to treat your condition.
• Will my insurance cover physical therapy?
It is important for you to be aware of what physical therapy coverage
your insurance plan provides. You can contact your
insurance provider and ask for your general physical therapy benefit.
It is usually expressed as a number of visits, a percentage of coverage,
and/or a copay. If they ask for specific codes (diagnostic
codes or treatment codes), let them know you simply need to know
your general physical therapy benefit. Mountain Town Rehab
is an independent physical therapy clinic.
• Does
Mountain Town Rehab participate with my insurance? Many insurance
plans do include coverage for physical therapy. You will be responsible
for any co-payments and/or deductible or any professional services
rendered that are not payable by your insurance company. We participate
with Blue Cross Blue Shield, Medicare, Medicare Advantage, McLaren
Health Advantage, Priority Health, AETNA, and most United Healthcare policies. We also participate
with the Cofinity Network, (formerly PPOM). Your "plan"
may have a different name, but the Cofinity Network may be listed
on your card. If you do not see your insurance company here, please call us, or your insurance company directly, to discuss coverage options.
• What do I need to bring for my first
visit? Your initial paperwork is available on this website,
under the tab Patient Info. You can print it, fill it out and bring
it with you for your first appointment. If this is not convenient
for you, you can arrive 15 minutes early for your appointment and
you will have time to complete your paperwork in the clinic before
your therapist is ready for you. You will also need to bring the
referral for physical therapy from your doctor, as well as your
insurance card.
• What should I expect at my first
visit? Your therapist will see you for your initial evaluation
which will take approximately an hour. Your therapist will ask about
your medical history, your current condition, your pain and limitations
and how this impacts your daily activities. Your therapist will
discuss your medications, tests, and procedures related to your
current condition. They will discuss your goals for physical therapy.
Treatment is usually initiated at this time.
• What
should I wear to therapy? You should wear loose fitting clothes
that allow access to the area requiring treatment; shorts if you
have a knee injury, a tank top if you need shoulder treatment, and/or
a loose fitting shirt and pants if the low back area will be treated.
Your comfort is very important.
• How long is each
session? Your initial evaluation will last approximately
an hour and your subsequent treatments will usually last from 45-60
minutes.
• How do I pay my copay? You will
be billed after your insurance company has paid their portion.
You will be responsible for any co-payments and/or deductible or
any professional services rendered that are not payable by your
insurance company.
